RESUMO

Disability is a problem that affects more and more children and adolescents. But in the course of our graduate and postgraduate education, we have received practically no training in this regard. A pediatrician is the primary care physician of every child and adolescent, and the above-mentioned lack of training becomes a hurdle in the care provided to children with disabilities. Our idea of diversity is clearly determined by our culture and the social and family environment where we grew up, and is highly influenced by our human and extracurricular development rather than by our medical training. Every course involved in professional training should include a subject on disability. As pediatricians, we should reflect on how we see children with disabilities.


La problemática dela discapacidad afecta aunnúmero creciente de niños, niñas y adolescentes. No hemos recibido, durante nuestra trayectoria académica como médicos, tanto en el pre- como en el posgrado, prácticamente, ninguna formación sobre el tema. El pediatra es el médico de cabecera de todos los niños, niñas y adolescentes, y la citada falta de formación afecta nuestra tarea para con niños con discapacidades. Nuestra concepción de la diversidad está claramente determinada por la cultura y por el medio ambiente familiar y social en el que nos hemos desarrollado y está muy condicionada por nuestra formación humana y extracurricular, más que por la educación médica. Se deben integrar contenidos sobre discapacidad en todas las unidades académicas responsables de la formación profesional. Los pediatras nos debemos una seria reflexión acerca de nuestra mirada sobre los niños con discapacidades.

RESUMO

La asociación entre la lipodistrofia congénita generalizada y la anomalía de Dandy Walker no es habitual. Se reporta el caso de una niña de 1 año de edad que ingresa al hospital a los 4 meses por riesgo social, con diagnóstico de anomalía de Dandy Walker. Durante su internación, se evidencia en forma progresiva aspecto acromegaloide, facies triangular, hirsutismo, lipoatrofia, hipertrofia muscular, clitoromegalia, distensión abdominal con hepatomegalia progresiva e hpertrigliceridernia. Se arriba así al diagnóstico clínico de lipodistrofia congénita generalizada. Se revisan los aspectos clínicos y el seguimiento interdisciplinario para la detección oportuna de insulinorresistencia y diabetes, pubertad precoz, miocardiopatía, entre otras. Respecto de la anomalía de Dandy Walker, se realizan controles evolutivos en búsqueda de la aparición de signos de hipertensión endocraneana. Por el carácter autosómico recesivo de la lipodistrofia congénita generalizada, es importante realizar el asesoramiento genético a los padres.


The objective of this study is to describe the unexpected association between the congenital generalized lipodystrophy (CGL) and Dandy Walker anomaly. Due to its autosomal recessive nature, it is important to provide genetic counseling to the parents.
